Job summary

University Hospitals Birmingham Foundation Trust provides Primary Care Services from Selly Oak Health Centre to the local population. As part of our transformation work with the Practice we are expanding our clinical nursing service and are looking for an experienced practice Nurse to lead this work and support the quality governance agenda to ensure the delivery of safe and effective care to our patients. If you have experience of leading staff, working as apart of a MDT and have a wide range of highly developed clinical skills with the ability to work autonomously we would love to hear from you

This is a fantastic opportunity for an experienced Practice Nurse to further grow your clinical and leadership skills whilst working as part of an innovative Out of Hospital Division within the Trust. You will be part of a diverse but supportive Divisional nursing team with opportunities to network across the wider Trust.

Main duties, tasks & skills required

In this role you will lead the provision of high quality nursing services to patients, including screening, assessment, treatment and advice, in order to promote and facilitate a healthy lifestyle and provide preventative health care.

You will lead on infection control in collaboration with non-clinical colleagues to meet the standards required of the practice, including monthly and annual audits. As part of the role you will also e required to lead in the development and delivery of specialist primary care services, such as long term disease management and ensure that National Service Frameworks (e.g CHD, Diabetes, mental health) are being delivered.

You will lead the nursing team to meet the needs of patients, working collaboratively with the general practice team and divisional team to support the delivery of policy and procedures, and provide nursing leadership. You will co-ordinate all available nursing resource, working with the practice manager and GPs to deliver the practice priorities and be able to assess effectiveness of care delivery for the nursing team through peer review, benchmarking and formal evaluation
