Job summary

We are looking for an experienced nurse to lead our successful Complex Discharge Team at Birmingham Heartlands hospital as part of our Out of Hospital division. You will be instrumental in supporting the discharge to assess process ensuring that there are no delays to discharge as part of our 'home first' approach.

We are looking for someone who has an innovative approach whilst constantly seeking better ways of working to improve outcomes for patients. You will need to have extensive experience in leading and managing multi-professional teams, a knowledge of community services and be able to form effective partner relationships within and outside of the Trust

Main duties, tasks & skills required

You will provide professional leadership, leading the operational and strategic day to day management of the Discharge Hub. You will work as part of the wider team that supports hospital discharge and will be part of a multidisciplinary and multi-agency team.

You will have a highly visible presence and ensure that the needs of the patients are placed on the centre of care delivery. You will promote best practice which inspires and motivates others, empowering them to ensure all staff, within their areas of responsibility, have the authority and support to provide agreed standards of care and service delivery to patients and their carers.

Working in partnership with social care staff the post holder will deliver corporate responsibilities across the whole of their portfolio, working collaboratively across organisational and professional boundaries, supporting the delivery of key objectives and strategic goals that drive quality assurance and improvement.

You will provide forward-thinking clinical and professional leadership to staff ensuring that discharge planning is delivered to a consistently high standard. You will contribute to the on-going implementation of Trust wide practice and quality improvement and will undertake a key role in ensuring the Division and its services meet the quality, activity and financial targets through effective planning, delivery, service improvement and management of performance improvement.

Qualifications

Essential

*Registered Health Care Professional on the NMC/HCPC register

*Evidence and ability to revalidate as required by professional body

*Evidence of professional/academic development to degree level or equivalent

*Management qualification/leadership course or equivalent experience

Experience

Essential

*Extensive experience in managing complex discharge and assessing patients for continuing health care requirements

*A proven track record of operational service management at a senior level in an acute hospital, including staff management, financial management and change management

*Proven ability to analyse complex problems, develop and implement practical solutions to address them.

*Ability to think and plan strategically and tactically and prioritise work programmes in the face of competing demands.

*Demonstrate a strong sense of personal and team accountability with a clear understanding of boundaries around delegated authority

*Ability to collaborate constructively with internal and external stakeholders and create conditions for successful partnership working.

*A good understanding of the NHS changing environment and challenges around cross boundary working
