Job summary

We are seeking enthusiastic Physiotherapists or Occupational Therapists to join our Community Therapy Team at Solihull Hospital. There is scope for successful candidates to consider full time hours, part time hours, flexible hours and/or a job share.

We are looking for therapists with excellent communication skills and a passion for Community based care, to join the team at an exciting time as we continue to align the Solihull Community Therapy Teams into one large integrated Community Hub.

The successful candidates will work as part of a well-established and respected team, assessing individuals and promoting early supported discharges, providing rehabilitation in own home. We work holistically, providing person centred care including interdisciplinary working to achieve positive outcomes for our patients

Ideally, candidates would have approximately 12-months post qualification experience, with a flexible approach and the ability to work autonomously, as well as in a team. Excellent organisational and time management skills are essential

You will need to have full driving licence, access to a vehicle and business insurance.

The Trust is committed to providing a 7 day therapy service. This role includes participation in a weekend rota.

Main duties, tasks & skills required

Solihull Community Therapy Team provides therapy services for patients across the community, those being discharged from acute hospitals, referrals in from GPs and other secondary care facilities.

We strive to provide a comprehensive holistic assessment within the individuals home, including those with diverse or complex presentations/multi pathologies; use advanced clinical reasoning skills and assessment techniques to provide an accurate diagnosis of their condition and / or problems, to then deliver individualised treatment programs.

The role will also include developing triage skills for incoming referrals within the developing Solihull Community Hub. Solihull Community Therapy Team also supports the Rapid Response Nursing service, striving to avoid hospital admission / in response to potentially complex situations.

There is a regular training program within Solihull Community Therapy Team available for the successful candidates to attend and participate in, alongside training from wider teams.

Person specification

Qualifications

Essential

Good standard of general education including, GCSE English and Maths grade C or above A Levels or equivalent qualification

Degree / diploma in Physiotherapy

Registered with Health and Care Professionals Council

Evidence of CPD including recent post graduate training relevant to clinical speciality

Experience

Essential

Demonstrates clinical experience enabling the post holder to manage a caseload of patients with complex needs. This would usually be approximately 12 months and will include experience in Medical, Older People, Trauma and Orthopaedics
Specialist knowledge across a range of practices, routine and non-routine, requiring expertise within the field of
Physiotherapy relating to Community Services
Previous and on-going attendance at mandatory training
CPD portfolio
