The Urgent Community Response Team aims to deliver holistic assessments and interventions to individuals who are at risk of hospital admission within their own home within 2 hours of referral to the service. The aim of the team is to enable people to stay at home, recover, and re-engage with the important aspects of their life and prevent admission to acute sectors
The post holder will work in the Urgent community response team and Virtual Wards to assist in the prevention of inappropriate hospital admissions and the facilitation of hospital discharge in line with the overall objectives of the service.
The post holder will ensure access to Urgent Health Assessments, provide and monitor short term treatment programmes at home and ensure safe discharge or referral to long term health and social care interventions. Utilising clinical expertise, clinical judgement, discretion and decision making in the development of individualised treatment.
To work in partnership with primary, secondary and other health and social care providers, to ensure patients receive appropriate investigation, intervention and treatment with minimal avoidable delays, whilst improving the quality of care.
To put the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, improving the quality of care.
The post holder will be expected to work within a flexible working pattern over a 7 day week in the urgent community response team as required.
Participate in the development and implementation of local nursing /therapy strategies and policies. for patients on a UCR pathway or on a virtual ward, Act as an expert practitioner, professional role model and specialist resource to the multi-disciplinary team regarding UCR /Virtual Ward
Actively promote participation by the team in current quality initiatives. Manage own time effectively and prioritise workload. Facilitate teaching and supervision for less experienced members of staff and students
Promote health by collaborative working with patients and their relatives. Safeguard individuals from abuse by reporting any incidents involving potential or actual abuse and acting upon concerns immediately. Participate in the further development of an integrated, multi-disciplinary Urgent Community Response Service working alongside colleagues in Integrated Multidisciplinary teams and the wider health and social care economic-ordinate other members of the Team.
